Colorectal carcinoma is one of the malignancies with a substantial global disease burden. Systemic inflammatory responses play an important role in its development and progression. The neutrophil-to-lymphocyte ratio (NLR) is an inflammatory parameter obtainable from routine blood testing and may be associated with clinical characteristics of colorectal carcinoma. This study aimed to determine the association between NLR and clinical stage among colorectal carcinoma patients at Dr. M. Djamil Hospital, Padang, in 2022–2023. This analytical observational study used a cross-sectional design. A total of 141 patients were selected using simple random sampling based on predetermined inclusion and exclusion criteria. Data were obtained from medical records, including patient characteristics, the first hematological examination at diagnosis, and clinical stage. Clinical stage was categorized as early (I–II) or advanced (III–IV), while NLR was categorized using a cut-off value of 4.79. Data were analyzed using univariate and bivariate analyses with the chi-square test. Most patients were male (51.00%), aged ≥50 years (65.95%), and diagnosed with stage III disease (43.33%). Of 141 patients, 76 (53.90%) had early-stage and 65 (46.10%) had advanced-stage disease. In the normal-NLR group, 42 patients (29.78%) had early-stage and 24 (17.02%) had advanced-stage disease. In the high-NLR group, 34 (24.11%) had early-stage and 41 (29.08%) had advanced-stage disease. A statistically significant association was found between NLR and clinical stage (p=0.022). Therefore, NLR was associated with clinical stage among colorectal carcinoma patients at Dr. M. Djamil Hospital, Padang, in 2022–2023.
