Abstract

Background: Chronic Obstructive Pulmonary Disease (COPD) is a chronic lung diseasecharacterized by progressive airflow obstruction and is often accompanied by the accumulation of secretions, resulting in ineffective airway clearance. One of the non-pharmacological interventions that can be performed to assist in removing secretions is chest physiotherapy, This research aims to determine the implementation of chest physiotherapy on the effectiveness of airway clearance in COPD patients. Methods: This research uses a descriptive analysis design with a case study approach involving wo COPD patients experiencing ineffective airway clearance nursing problems at TK IT Putri Hijau Hospital, Medan. Data were collected through interviews, observations. physical examinations, and documentation. Nursing care was carried out through the stages of assessment, diagnosis establishment, intervention, implementation, and evaluation. The implementation of chest physiotherapy accompanied by effective coughing was performed on COPD patients for approximately 5-10 minutes over 3x24 hours (3 days) in accordance with the Indonesian Nursing Intervention Standards (SIKI), Results: Prior to the research, patient 1 had a respiratory rate (RR) of 25 breaths/minute, oxygen saturation (SpO:) of 92%, and sputum production of 20 mL/day. Patient 2 had an RR of 29 breaths/minute, SpO, of 95%, and sputum production of approximately 20mL/day, After the intervention. patient 4 showed an RR of 22 breaths/minute, SpO, of 09%, and sputum production of approximately 5 mL, while patient 2 showed an RR of 22 breaths/minute, SpO: of 99%, and sputum production of approximately 5 mL. The resus of the research show that the problem of ineffective airway clearance was resolved in both patients after the implementation of chest physiotherapy and effective coughing. Conclusion: This intervention can be used as one of the independent nursing interventions and as a non-pharmacological therapy to improve effective airway clearance in COPD patients.
