Post-dural puncture headache (PDPH) is one of the complications of spinal anesthesia that may occur following dural puncture. This study aimed to describe the occurrence and characteristics of PDPH in patients undergoing spinal anesthesia at Dr. M. Djamil General Hospital, Padang. This descriptive observational study employed a purposive sampling technique and was conducted from February to March 2024. Data were analyzed using univariate analysis. Of the 42 patients who met the study criteria, 11 developed PDPH, resulting in a proportion of 26.2%. PDPH was most frequently observed among patients aged 21–39 years, females, those with normal body mass index (BMI), L3–L4 puncture, 25G spinal needle use, and non-obstetric procedures. The predominant pain characteristics were mild intensity, frontal location, and onset 48 hours after the procedure. However, the frequency of PDPH varied across several categories, particularly BMI and type of procedure, with some findings differing from those reported in previous studies and established theories. In conclusion, PDPH occurred in more than one-quarter of the patients undergoing spinal anesthesia in this study, with characteristics varying according to patient and procedural factors
