The high prevalence of smoking among older adults remains a public health issue to this day, given that this group is highly vulnerable to various health complications. Knowledge about the dangers of smoking is a key factor influencing smoking behavior. The purpose of this study was to determine the level of knowledge about the dangers of smoking and smoking behavior among older adults in RT 09 RW 09 Kramat Jati, East Jakarta. This study employed a quantitative descriptive method using a survey approach involving 32 elderly smokers. Data were collected using a questionnaire to measure knowledge levels and smoking behavior. The results showed that 16 respondents (50%) had a high level of knowledge about the dangers of smoking, while the majority—20 people (62.5%)—exhibited high levels of smoking behavior. This study indicates that a high level of knowledge does not always align with healthy smoking behavior among the elderly, a phenomenon attributed to various factors. The study’s findings can serve as a basis for designing educational programs for elderly smokers to raise their awareness of the dangers of smoking and improve their smoking behavior.
